Form R-44

Form R-44 is the RARS.NET General Purpose Request and Inquiry Submission Form. It is used for service inquiries, incident reports, data access requests, reticulation complaints, compliance questions, and any other matter that does not have its own dedicated form, which most matters do, though the dedicated forms typically require Form R-44 as a prerequisite.

Before You Begin

Please read all instructions carefully. Incomplete submissions will be returned without processing and cannot be resubmitted for 14 business days.

Instruction 1 — Eligibility

Form R-44 may be submitted by registered endpoint operators, authorized representatives of registered entities, individuals with a pending registration application (Form E-22), and members of the general public who can demonstrate a "reasonable nexus" to RARS.NET services. Reasonable nexus is defined as physical presence within 50km of a registered endpoint, receipt of synthesis-adjacent telemetry, or the ability to articulate why your matter involves RARS.NET to the satisfaction of the intake reviewer.

Instruction 2 — Required Information

All fields marked with an asterisk are required. Fields not marked with an asterisk are optional but may be required by the intake reviewer during processing, at which point the submission will be placed on hold until the information is provided. Fields left blank will be interpreted as "not applicable," "unknown," or "declined to state," each of which carries different processing implications that are not disclosed in advance.

Instruction 3 — Supporting Documentation

Submissions may require supporting documentation depending on the nature of the request. Acceptable documentation includes: endpoint registration confirmation, regional authorization letters, incident reference numbers, photographs (non-digitally-altered), notarized statements, and any RARS.NET correspondence received within the last 24 months. Documentation older than 24 months must be accompanied by a Form D-12 (Document Recency Waiver), which is itself submitted via Form R-44.

Instruction 4 — Processing Times

Standard processing time is 6-8 weeks from receipt of a complete submission. Submissions requiring inter-regional coordination may take 10-12 weeks. Submissions involving contested regions, disputed endpoints, or matters that reference Region 8 may take 12-16 weeks. Expedited processing is not available. Inquiries about processing status are themselves processed as new Form R-44 submissions and are subject to the same timelines.

Instruction 5 — Submission Limits

Each individual or entity may submit a maximum of three (3) Form R-44 submissions per calendar quarter. Submissions that are returned due to incompleteness count toward this limit. Submissions that are consolidated with other submissions by the intake reviewer count as one submission for the submitter and one submission for each party whose matter was consolidated, whether or not they were aware of the consolidation.

Form R-44 — General Request

Must match the name on your endpoint registration, if applicable.
If unknown, leave blank. An intake reviewer will attempt to determine your endpoint.
Region 8 selections will be flagged for additional review.
Telephone is not an available contact method for Form R-44 responses.
"Existential Concern" is for matters related to the nature or purpose of synthesis operations. Personal existential concerns should be directed to the Employee Wellness Hotline.
Maximum 2,000 characters. Descriptions exceeding this limit must be submitted as an attachment via Form D-12.
Found on incident notification letters or the public Incident Archive.
Follow-up submissions count toward your quarterly limit.

By submitting this form, you acknowledge that: (a) the information provided is accurate to the best of your knowledge; (b) inaccurate information may result in processing delays, submission rejection, or referral to the Compliance Office; (c) the act of submission constitutes consent to the collection, retention, and processing of the submitted information under the terms of the RARS.NET Privacy Policy and Addendum B; (d) submission does not guarantee a response, resolution, or acknowledgment beyond the automated receipt confirmation, which is itself subject to delivery conditions outside RARS.NET's control; and (e) you have read and understood the instructions above, or accept responsibility for any consequences arising from having not done so.

What Happens After Submission

Upon receipt, your submission will be assigned a reference number and routed to the appropriate regional intake queue. An intake reviewer will assess the submission for completeness, relevance, and routing eligibility. Submissions that pass intake review are forwarded to the applicable department, which will process the request according to its own internal timeline, which may differ from the estimates provided above.

You may check submission status by submitting a new Form R-44 with "Nature of Request" set to "General Inquiry" and the prior submission reference in the appropriate field. Status inquiries are processed at the same priority as the original submission, meaning you will typically receive a status update at approximately the same time you would have received the original response.
